No more co-pays or hidden fees! We operate on a membership model, often priced like a monthly cell phone bill, including many labs at no extra cost and offering discounted labs.
No more waiting for weeks to see your provider! You have direct, 24/7 communication with providers (text, call, or email) and same or next-day appointments.
No more seeing providers who do not know you! We want to know you so that we can better care for you.
No more feeling rushed in the exam room to talk about your concerns! You have extended, unrushed visits if needed (30-90 minutes).
No more being treated like you are a symptom or disease! Your health is important and the source of your problem is too.
- Direct Access to your medical provider, nearly 24/7 via phone, text, & e-mail.
- 30 - 60 minute Visits are the norm, but more if needed.
- Same-day or next-day appointments for Acute Concerns.
- Many Labs are included; otherwise, we offer Wholesale lab prices (Ask bout our low-Cost Labs).
- Ordering Prescriptions, Imaging, or Referrals if needed.
- For the big things that are not covered in DPC, you can use your health-share, insurence or pay out of pocket, and we are always working to connect our patents with more affordable options.
- Management of Chronic, long term, conditions. which includes treating symptoms, monitoring the condition, exploring further into your conditions if needed, and coordinating care with specialists.
- Personalized Evaluation and basic Wellness/Lifestyle recommendations.
- Preventative Health Physicals and/or Well-child check ups.
- Many In-office tests and procedures are included at no extra charge, including EKG, skin excisions (mole removal), and Some cosmetic procedures to name a few (talk to us for details).
- Home Visits for DPC members
- $30 additional per home visit.
- The decision whether a home visit is appropriate is up to the clinician’s discretion and availability.
- Home visits are limited to no greater than 15 miles drive from the following location: 217 Moreland Ave W, West St Paul, MN 55118.
There is no initial Enrollment fee.
We take HSA and FSA. It is the responsibility of the patient to follow IRS guidelines.
The Family clinic membership has no long-term contract or
deductible.You can cancel your membership at any time with 30 days notice.
There is a $150 re-enrollment fee for those who wish to rejoin after cancelling membership (based on availability).
We Take care of about 90% of your care!
