DPC Membership Pricing

Home Visits
- Home Visits Only Per Clinician’s Discretion.
- Home Visits are and additional cost of $40/visit . This is a per visit fee that is in addition to the monthly membership fee.
- Home visits are limited to no greater than 15 miles drive from the following location: 217 Moreland Ave W, West St Paul, MN 55118.
- Specific Services Included in DPC Membership are Described in Greater Detail in the Membership Agreement.
Medicare
- We do not accept Medicare and currently we are unable to see Medicare patients in Direct Primary Care or in Weight Management.
- In the future we may accept Medicare patients; if interested please call, and we can place you on our wait list.
Medicaid/Minnesota Healthcare
- At the time this was written, it appeared that DPC memberships were not yet a covered service.
- Please call your Minnesota Healthcare program for questions about what is covered.

- Unlimited Telemedicine Visits.
- Unlimited In Office visits.
- Home Visits as Below:
- Two home visits are included per household per year for members.
- Any Additional Home Visits are a $10.00 fee and an additional $1/mile per visit.
- Home visits are limited to no greater than 20 miles drive from the following location: 217 Moreland Ave W, West St Paul, MN 55118.
Specific Services Included in the Membership are Described in Greater Detail in the Membership Contract.
